Further enhancement to mini songbook

I have added a bit more to the mini songbook. The songbook Table of Contents now contains page numbers and is in 2 columns (as the new full songbook will have).

There is now a cover page that contains a text box that you can enter text into. On the Faves page there is an input box down the bottom with this text in front of it:

Enter text for cover page (180 chars max and only for Merge Song PDFs): 

You can enter up to 180 characters into this box and they will appear on the cover page.

This could be useful for clubs who want to create an event songbook.

A date will be added after the cover page text
